Abbreviated Information:

A sterile disposable device consisting of a Karman-type suction curette for use with 60cc Handyvak Locking Syringe. The Cannula-Curette provides a quiet portable device for obtaining a histologic sample from the endometrium on an outpatient basis usually without anesthesia.

The smaller sizes are especially useful in early detection of endometrial carcinoma or its precursors and endometrial dating for infertility.

Purpose:

1. Diagnostic: Obtain clearly differentiated endometrial tissue usually without anesthesia in an office setting. Diagnostic accuracy of histology of the endometrium is far superior to that of cytology.
2. Therapeutic: Empty contents of uterine cavity:
   1. Following spontaneous or incomplete abortion
   2. Prior to hysteroscopy
      1. To clear the visual field in patients with uterine bleeding
   3. Menorrhagia and metrorrhagia
   4. Prior to Endometrial Ablation

Indications:

1. Detection of endometrial hyperplasia
2. Detection of endometrial cancer
3. Endometrial dating
4. Initial endometrial biopsy prior to prescribing estrogen replacement therapy to ensure endometrial lining is normal
5. Periodic monitoring of endometrial tissue in women receiving estrogen replacement therapy
6. Examination of glandular epithelium to determine ovulation and endometrial response to hormonal excretion and therapy
7. Detection of pathology resulting in infertility
8. Endometrial biopsy for postpartum endometritis
9. Bacterial culturing
10. Monitor patient receiving TAMOXIFEN therapy for breast cancer
11. Secondary amenorrhea
12. Following spontaneous abortion

Advantages:

• When no suction pump is available, Cannula-Curette with 60cc Handyvak Syringe is an ideal low cost device available in stainless steel (2-3mm) or flexible plastic and nylon semi-rigid curettes in sizes 3mm thru 7mm
• Ideal in settings where noise of suction pump may be objectionable
• Sizes may be assorted (no minimum per one size required) to allow for the widest range of uses

Limitations:

Small lumen curettes are contraindicated in patient with dysfunctional uterine bleeding. Clots can clog curette. Small lumen curettes are contraindicated in patient with large cervical os because there is the loss of suction around the curette.

Larger sizes recommended for curettage of uterine cavity:

• Following spontaneous incomplete abortion
• Prior to hysteroscopy and endometrial ablation to clear visual field in patients with uterine bleeding.
• Menorrhagia
• Metrorrhagia
• Cannula Curettes with syringe sizes may be assorted (no minimum per one size required) to allow for the widest range of uses

Contraindications: Contraindicated in the presence of any vaginal, cervical, and/or pelvic infection(s). Infections must be treated prior to any endometrial curettage or biopsy.

In general, the same criteria for regular hospital D & C should be followed for office suction curettage.
